Required Forms for Full Club Registration
After your initial club registration has been received, clubs will need to also complete the below 4 documents and submit the forms to Athletics. To use template, click file -> make a copy and save in your own google drive.

Club Sport Budget Template (google sheet template > copy & save to edit)
Club Sport EAP Template (google doc template > copy & save to edit)
Travel Forms
Travel Request Form (Google form – due 4-6 weeks prior to travel based on in country or out-of country. See page 10/11 of Handbook)
Travel Registration Package (e.g. Roster & Itinerary) (downloadable PDF – due 3 weeks prior to travel)
